Actions: To replenish Yin in the liver and kidneys.

Indications: blurred vision, pain of the eyes, dry eyes, irritated epiphora, dull headache and severe dizziness, ear ring and deaf, hectic fever and night sweat, due to Yin deficiency of the liver and kidneys.*

杞菊地黄丸方解：

中医认为，肝开窍于目，肝血上注于目则能视，即眼睛的功能与肝密切相关；在五行理论中，肝属木，肾属水，水能生木，肾与肝是一对母子关系，即肝为肾之子，肾为肝之母，母脏病变会影响到子脏；又肝主藏血，肾主藏精，精、血互生，因此肝与肾密切相关；因此，治疗眼部疾病，往往从肝肾入手。

杞菊地黄丸由六味地黄丸加枸杞子、菊花而成。枸杞子：甘平质润，入肺、肝、肾经，补肾益精，养肝明目；菊花：辛、苦、甘，微寒，善清利头目，宣散肝经之热，平肝明目。八种药物配伍组合共同发挥滋阴、养肝、明目的作用，对肝肾阴虚同时伴有明显的头晕视物昏花等头、眼部疾患，尤为有效。

药理研究：

可增强免疫功能，抗衰老，改善肝脏脂肪代谢，促进肝细胞新生，预防脂肪肝发生，降低毛细血管通透性，抗炎，降低四氧嘧啶引起的高血糖，减少东莨菪碱对学习记忆功能的影响，抗肿瘤、降血脂。

临床应用：

对于因肝肾阴虚引起的视神经炎、球后视神经炎、视神经萎缩、中心视网膜炎、眼干症、慢性青光眼、老年性白内障、早期老年黄斑变性等眼部疾患均有明显的治疗和改善症状作用。

Ingredients: Sickle-pod senna (seed), Uncaria rhynchophylla (stem), Achyranthes (root), Gastrodia (rhizome), Gardenia (fruit), Chinese skullcap (root), Eucommia (stem bark), Siberian motherwort (above ground parts), Loranthus (twig and leaf), Fo-ti (dried root tuber), Poria (sclerotium)

Actions: Suppresses hyperactivity of liver Yang.

Main indications: Use for hyperactivity of liver yang, upward attack of liver wind, headache, vertigo, and insomnia, manifesting a red tongue with yellow coating and wiry pulse. Often used for hypertension and acute cerebrovascular illness.*

Directions: Take 8 pills each time, 3 times a day.

Notes: This modern preparation is based on a classical prescription, Tianma Gouteng Yin (Gastrodia and Uncaria Decoction), from Zabing Zhengzhi Xinyi (New Significance of Patterns and Treatments in Miscellaneous Diseases, the best-known traditional prescription with gastrodia from the Qing Dynasty. This formula is used primarily for treatment of headaches and dizziness, mainly in cases of high blood pressure.

Source: The tuber of astrodia elata Bl. (Orchidaceae) symbiosis with Armillaria mellea (Vahl ex. Fr.) Quel. (Fungi), boiled with water or steamed and dried after the cortex is discarded.

Characteristics: The tuber appearing as long oval in shape, circular joints with spotted root scars over the cortex and a "parrot-beak" like bud at the top of the tuber or stem base. On the cross section, the cutin appears translucent without any fibre. Sweet in taste, mild in nature and attributive to liver channel.

Indication:

1. Calm the liver-wind: For syndrome of liver-wind stirring inside, such as infantile convulsion, tetanus, epilepsy, as well as dizziness and headache due to asthenia of liver-yang or the attack of wind-phlegm. Recently it is also used for the treatment of neurasthenia, nervous headache and hypertension.

2. Expel wind evil and alleviate pain: For migraine, arthralgia due to wind-dampness type, numbness of the extremities, and general fatigue, etc.

Pharmacological Action:

Its active component gastrodin exerts as sedative, hypnotic and antispasmodic.
It may increase blood flow and decrease the peripheral resistance of blood vessels.

功能与主治：平肝熄风，清热活血，补益肝肾。用于肝阳偏亢、肝阳化风、肝风上扰。症见高血压，头痛，晕眩，失眠。

用法与用量：每次8粒，每日3次。

现代应用：本方来自《杂病症治新义》天麻钩藤饮。现代常用于高血压病、急性脑血管病、内耳性眩晕等属于肝阳上亢，肝风上扰者。

治疗高血压的中药很多，但也需辨证施治，可用单方，也可配伍使用。现介绍如下：

1、罗布麻：味淡涩性惊，有平肝降压、清热利尿作用，用于肝阳上亢或肝热型高血压症，见于头晕头痛、烦躁失眠者，可单用本品以开水泡汁作饮料，也可配合夏枯草、野菊花、钩藤同煎。
2、稀莶草：味苦性寒，有祛风通络、清热降压作用，可同臭梧桐等配伍，用于高血压及风湿病。
3、臭梧桐：味辛苦性凉，有祛风湿、降血压作用，与稀莶作用相似。本品降压，用开花前的叶作用较佳，不宜久煎。
4、夏枯草：味辛苦性寒，有清肝、散结、化痰、降压作用。由于本品有良好清泄肝火作用，可泡服或煎服，用于高血压病属肝热、阳亢者。
5、钩藤：味甘性微寒，具清热平肝、息风止痉作用，对高血压引起的头胀头痛症状疗效较好。
6、杜仲：性味甘温，有补肝肾、强筋骨、安胎降压功效，与石决明、夏枯草、白芍等配伍可降血压，且对体虚之人降血压无寒凉之弊。
7、决明子：性味甘寒，清肝明目，润肠通便，有降低血脂及血压功效，对防止血管硬化有效，尤适于兼有便秘的中老年患者。
8、石决明：味咸性寒，有清肝明目、平肝潜阳功效，与生地、白芍、钩藤、菊花等配伍善尤其适用于高血压头晕头痛者。
9、茺蔚子：本品为益母草的果实，味甘性微寒，能活血调经、凉肝明目和降血压，常与决明子、生地、钩藤等配用。

中医根据辨证论治的原则，对不同类型的高血压选用相应的中成药治疗：

高血压病相当于祖国医学的“眩晕”、“头痛”、“肝风”、“肝阳”等。其病因为饮食不节，劳伤虚损等。病位在肝、肾，常累及心、脾，肝肾阴阳失调为其主要病机。故中医治疗本病，多极据不同的证型，投以中成药。

一、阳亢型

钩藤片由钩藤制成，具有平肝潜阳，降压之功效，可用于各期高血压。每次10－20毫克，每日3次。

降压糖浆由夏枯草、白芍、槐角等组成，具有清肝火、降血压之功效，用治高血压之头痛目眩，目赤耳鸣，心烦不眠。每次15毫升，每日3次。

罗布麻降压片由罗布麻、夏枯草、钩藤、牛膝、菊花、珍珠母、泽泻等组成，具有平静降压，镇静安神，强心利尿之功效。用治高血压病之头痛耳鸣，口苦心烦，少寐多梦等。每次4－6片，每日3次。

二、阴虚阳亢型

降压养血冲刺由珍珠母、桑椹、地骨皮、夏枯草、黄芩、野菊花、白芍、青木香组成，具有平肝潜阳，养血祛风之功效。用治高血压引起头痛头晕，心烦心悸等，每次1袋，每日3次。

降压丸由珍珠母、龙胆草、槐米、夏枯草、地黄、牛膝组成，具有降压作用，用治高血压之头晕目眩，耳鸣目胀，每次6丸，日3次。

三、阴阳两虚型

补肾丸由磁石、菟丝子、五味子、枸杞、地黄、石斛、复盆子、肉苁蓉、车前子、沉香组成，具有补肝肾，健脑明目之功效，用治气血两亏之头晕耳鸣，视物不清，腰膝酸软等。每次20粒，每日两次。

血平片由毛冬青、桑寄生、夏枯草、旱莲草、槐米、黄精、谷精草、钩藤、黄芩、牛膝、珍珠层粉、升麻组成，具有平肝镇静，通络活血之功效，用治高血压、动脉粥样硬化等。每次3片，每日3次。

四、阳虚型

金匮肾气丸由熟地、山药、山茱萸、茯苓、丹友、泽泻、肉桂、附片、淮牛膝、车前子组成。具有温补肝肾，化气行水之功效。用治脾肾阳虚、痰饮、腰痛、足肿、尿频。每次1丸，每日两次。

杜仲酊由杜仲组成，用治高血压。每次1－2毫升、每日3次。

五、风痰瘀兼发证

舒心降压片由郁金、丹参、红花、葛根、桃仁、槐米、钩藤、菊花、牛膝、柏子仁组成，具有活血化瘀，舒心降压之功效，用治原发性高血压、动脉粥样硬化、冠心病等。每次6－8片，每日3次。

牛黄降压丸由牛黄、珍珠、冰片、黄芪、郁金组成，具有清心化痰，镇静降压之效。用治肝阳上亢，痰涎壅盛之头晕目眩，性情烦躁等。每次1－2丸，每日1次。

治疗高血压病的中成药：

(1)龙胆泻肝丸具有清肝火，泻湿热的作用。适用于年龄较轻，病程较短，见头痛、头胀、头热、小便短赤、舌红苔黄等肝经实热的高血压病。按肝火症状的轻重适量服用。口服每次6～9克，日服2～3次。

(2)当归龙荟丸具有清肝泻火、通便导滞的作用。适用于体质壮实，面红目赤，烦躁不安，大便秘结，头痛头晕较剧，甚至呕吐抽搐等肝火较盛的高血压病。每次6克，日服2～3次，饭后温开水送服。

龙胆泻肝丸和当归龙荟丸，皆有清肝泻火治疗高血压的作用，这是它们的相同点，但也有不同之处，前者泻湿热从小便而出，方药的组成泻中有补，作用较缓和。而后者使湿热从大便而泻，药性大苦大寒，泻火通便作用较强，然非实热症急的高血压病不可轻用，孕妇禁忌。

(3)脑立清具有镇肝潜阳降逆作用，用于气血上逆的头目眩晕，头痛脑胀的高血压病，每次10～15粒，口服2～3次，饭后温开水送服。

(4)清脑降压片具有滋阴清肝，潜阳降压的综合作用，适用于头目眩晕，失眠烦躁，耳鸣耳聋，舌红少苔等肝阴虚，肝火旺的高血压病。口服每次4～6片，孕妇禁忌。

(5)杞菊地黄丸具有滋肾阴，清肝热的作用。适用于肾阴虚引起的头晕眩晕，眼花目涩，五心烦热，腰膝酸软，年老体弱，病程较久的高血压病。每次9克，日服2次，适用长期服用。
